Printing and mailing application/recommendation forms (UCA)

UCA member colleges prefer to receive forms online.

If it is not possible to submit forms electronically, PDF copies of blank UCA forms are available for download at https://www.universalcollegeapp.com/resources.

Click the link for the form you wish to print. When the PDF opens, print it, complete off line and then mail to the specific institution directly. You can find the contact information of the institutions here: https://www.universalcollegeapp.com/schools.

You will not be able to track the progress of any forms completed outside of the online system in your Checklist.
